If you have inquiry about fee schedules, please contact our office and ask for an initial consultation. Please note that your actual insurance benefits will determine the out-of-pocket amount based on the conditions including the portion of your deductible already met and your co-pay amount.
We accept checks, debit cards, and all major credit cards (Visa, Mastercard and American Express) as valid payment methods. According to our policy, the fee must be paid at the time of your consultation or treatment session.
Please be aware that, as of now, Dr. Matsuki is not an in-network provider for any insurance panels. If your insurance plan offers out-of-network benefits, your claims will be submitted to your insurance company by our office for your reimbursements. Before any treatment, our office will provide an estimated financial responsibility, calculated from specific benefit levels on your active insurance coverage. Please feel free to contact us for assistance because we are always here to help. Also, if you need a pre-authorization prior to receiving our service, we will contact your insurance company.
You must provide us two business days’ (48 hours’) notice, to cancel of reschedule your session for any reason. For instance, if you have an appointment scheduled for a Monday, you are required to notify our office on or before the preceding Friday. If you fail to cancel and miss a session, or cancel a session with less than 48 business hours’ notice, you must pay the full amount of the appointment fee according to our cancellation policy.
